You are to design a database application from the ground up using the attached ER
Diagram on the next page as your guide. Here are specific instructions:
1. Use the Students table found in the Access file.
It has already been designed for you
and populated with data. Please design an effective user interface (i.e., form) that
populates data into the Students table. See lab tutorial on User Interface Design for
more information on this.
2. You are to implement the supertype/subtype relationship. There are three subtypes.
Please include the following subtype-specifc attributes in your subtype tables:1
Undergraduate: Major, HonorsStudent? (yes/no), StudentAthlete? (yes/no), GPA
Graduate: Degree (MS, MBA, MA, PhD), AreaOfStudy, GPA
Online: OnlineType (Full-Time, Part-Time), DegreeProgram (Certificate, BA)
There is no data for the subtype tables—you must populate the subtypes with sample
data. Moreover, the student table only contains information on undergraduate
students, so you must enter graduate and online students in this table as well.
3. You are to build forms that populate data into each of the tables on the ER Diagram.
I do not want you to enter the data directly into the tables, but via the forms. When
building your forms, please use appropriate Access user interface controls (e.g.,
checkboxes, list boxes, combo boxes, etc.). Again, see lab tutorial on User Interface
Design for more information on this.
4. You are to develop a report called Student Report Card (see attached sample). This
report will be based on a parameter-based query. A user will be prompted to enter a
Student ID and a semester (e.g. Spring 2020). The query will then pull data that satisfies
this criteria and generate a report card for the given Student ID and given semester.
You must assume that the Grades table has data for multiple semesters. Please try to
get your report to look like the sample.
Extra Credit if you calculate and display the GPA on the report. Please assume that
there are only five valid grades: A, B, C, D, and F (no pluses or minuses).

5. You are to develop a second report called Classroom Performance Report. This
report will also be based on a parameter-based query. A user will be prompted to enter
a Professor ID and semester. At the top of the report will be displayed professor’s full
name and contact information (email and phone number). The report will display all the
courses (both course number and course name) that a professor has taught for that
semester. Within each course, student names (both first and last names) will be
displayed together with a student’s grade. The student information will be grouped by
letter grade—all the A students will be grouped together, all the B students, all the C
students, and so on. At the bottom of each letter grade grouping, you will print out the
following:
Number of students receiving grade A: 4 (or whatever the count is for that letter
grade).
You will do the same for B, C, D, and F. If no student was assigned a particular letter
grade then you don’t need to include the count (i.e. 0).
6. Finally, please assume that a course, as given by the course number on the course
table, will only have one professor assigned to it. For example, AIMS 2710 is always
taught by one and only one professor (I know this is an unreasonable assumption, but it
will simplify your database design!).
