This is the introduction paragraph. Here you want to introduce your topic and grab the reader’s attention. Your introduction paragraph should be 4–6 sentences long and will include your thesis statement. Remember, the thesis statement states the main focus or main idea of the entire essay and is normally the last sentence in the introduction; however, more importantly, it should be obvious what your thesis statement is.

Nutrition Risk

This section should provide information such as:  Based upon review of the diagnosis, dietary recall, social/psychological profile of the client, medications, lab values, clinical observations, what are the nutrition risks for this client?  As an example, client might be at risk for a calcium deficiency due to age, gender, family history, low calcium intake, low BMI; client might be at risk for energy-protein deficiency due to living below the poverty level, poor PO intake, and having to feed 5 children; etc.

Nutrition Interventions

This section should include information such as: For each nutrition risk identified, make sure to include at least 1 intervention.  As examples: How will the client be able to reach the weight loss/gain?  What steps will need to be taken to decrease/increase the lab values?

Expected Outcomes

This section should include information such as:  For each nutrition intervention identified, make sure to include the expected outcome.   As examples: lab value with become stabilized due to adjustment in diet; weight loss will cease as a result of increased caloric intake; etc.

Goals of Nutrition Therapy

This section should include information such as:  What goals will be set for the client for the short term (3 months)?  What are the goals for the long term (9+ months)?   As examples: gaining/losing a certain number of pounds of body weight by the end of 3 months; increase/decrease in a lab value within 1 month; ability to write a menu for the dietary restrictions within 1 week; etc.

Dietary Modification

This section should include information such as:  Would your client benefit from a restriction in dietary intake?  How might this benefit the “diagnosis”?  What research would support this restriction?  What would the dietary restrictions look like?  Give some menu/food examples.

Nutrition Education Session

This section should include information such as: What will be the nutrition information covered?  Who will be involved in the nutrition education session?  Where will the sessions take place?  How will the session be organized to present information?

Teaching Method

This section should include information such as:  How will the information be presented to your client?  Examples:  lecture, discussion, videos, handouts, etc.?  What educational materials/resources will be used?  What additional resources would be appropriate for the client in the long term?  Examples: organization web sites, support groups, phone number for Registered Dietitian Nutritionist, etc.

Tip to Avoid Potential Herb/Nutrient/Drug Interactions

This section should include information such as:  What is one specific tip regarding potential herb/nutrient/drug interactions based upon medications identified and/or nutrients important for your client’s situation?

The final paragraph is the conclusion paragraph. This paragraph can be shorter than the others. Remember to wrap-up the essay for the reader. The conclusion should not introduce any new material. Finally, you will want to refer back to your thesis statement.
