Summative Assessment Guidelines Type: Written assignment Wordage: 3000 words (+/- 10%) [Citations within the main body of the assignment are included in the word count; reference lists are excluded.]

Title: Integrated Health & Social Care Learning outcomes: On completion of this module, students are expected to be able to:

1. Examine and evaluate integrated health and social care delivery in the context of current relevant policies.
2. Evaluate the characteristics of effective leadership and inter-professional working to inform personal and professional development.
3. Analyse the processes which impact on integrated health and social care working.
4. Appraise your own application of the principles of interagency and inter-professional working in order to promote integrated health and social care in practice.
5. Evaluate the attributes required of a competent professional practitioner to provide transformative care.

Key features of the assignment:

Critically evaluate the delivery of integrated health and social care with reference to contemporary policies and policy drivers (LO1).

Barriers and facilitators impacting on effective integrated health and social care delivery must be analysed (LO3) and the key attributes required to promote integrated working, deliver transformative care and lead effectively must be evaluated (LO2, LO4 & LO5).

Throughout the assignment professional, academic sources of literature and evidence must be used to support the discussion.

A minimum of 30 sources of this nature would be expected for a quality piece of work, and these must be referenced within the assignment.

Students are expected to demonstrate personal and self-directed reading beyond that of the provided course materials, and this should be reflected in your reference list.
