An appropriate discharge plan will ensure that Mr. Trosack is able to continue with treatment for non-insulin-dependent patient. The discharge plan is developed by the involved parties who mainly the physicians, nurses, certified diabetes educators (CDEs). The patient should be able to use the acquired skills for survival without the presence of a nurse of physician for aid in administering medication. Hence Mr. Trosack should be able to ensure that he is able to administer the test for glucose levels using the glucometer and understand the actions that he is supposed to take for low levels of glucose in the blood. His skill in conducting self tests warrants his release (Rorden, & Taft, 1990).
